Schools Plus Community Outreach Worker
7.00 hours per day

The Schools Plus Community Outreach Worker under the direction of the Annapolis Valley Schools Plus Facilitator would provide direct support to children, youth, and families and would assist in navigating children, youth and their families to services and resources in the community on a 12-month basis. The Outreach Worker would work collaboratively with other school and agency staff to ensure that the student and/or their family have timely access to services and programs required.
REQUIREMENTS FOR SCHOOLS PLUS COMMUNITY OUTREACH WORKER
The Schools Plus Community Outreach Worker will require the completion of a two year of Human Services Diploma, with a formal practicum placement and experience in a related human services discipline.
- Has the ability to work as part of a team with minimal supervision.
- Has discretion with confidential matters.
- Initiative, coupled with good judgment, is required to take appropriate, immediate action relative to the client circumstances and time.
- Ability to exercise good judgment with respect to changes in client/family circumstances and reports to the Community Outreach Facilitator.
- Ability to establish and maintain a caring, supportive and trusting relationship.
- Ability to coordinate and collaborate with other persons and services as part of a Comprehensive Plan of Services for a family.
- Ability to organize and carry out tasks efficiently as a team member and individually.
- Ability to work independently and respond appropriately to crisis situations.
- Ability to share verbally and narratively with the facilitator/supervisor/family.
- Direct knowledge and experience of the community served and services available is an asset.
- Experience in facilitating groups.
- Transportation of students.
- Must have a valid N.S. Driver’s License.
